    I found the problem. Maximum file upload size was too small to allow the specific file I was uploading. I increased the size and the file loaded fine. Better error messaging would have been helpful!

    I don’t believe that this issue should be marked as resolved.

    I’ve updated to latest WordPress and installed the latest Enable Media Replace, yet I too still receive this erroneous redirect to wp-admin/wp-admin with zero information that my file size was too large.

    As @kferrelli said – better error messaging please.

    Definitely don’t think this is resolved. I just received this for the first time with version 3.2.1. I could not get it to go to completion, even with the following max:
    @ini_set( ‘upload_max_filesize’ , ‘256M’ );

    It uploads to 99%, then:

    “404 – File or directory not found.
    The resource you are looking for might have been removed, had its name changed, or is temporarily unavailable.”
