Gallery plugin with 'request info' button option

  • Is anyone aware of a gallery plugin that allows for a ‘request info’ link or button below each image in a specific gallery?

    The ‘request info’ would link to a form that submits by email, and includes the image name

    It would also be great if each image would allow for a description, as well as the the ‘request info’ link/button

    All help greatly appreciated
