[NSFW] Frontend Registration

I installed your plugin to make Contact Form 7 a registration form. The plugin effectively makes it a registration form however there are problems because it puts the name in all the fields. Furthermore, fields like [description], [your-telephone][your-city][your-region][your-state][your-address] do not include them. What are the settings for everything to work correctly? Thank you
