Front end form for posting, categories question

I’m using wordpress for a “communications log” for my business. I would like users to be able to post messages from the front end without going to the editor. I would also like them to be forced to choose a “category” and have formatting choices. If I could have the form on the edit page on the front end that would be perfect.

Any suggestions would be appreciated.
