Frequent 404 errors

Over the past few months, we’ve been seeing increased 404 errors on our site when a visitor tries to access a /download URL. We’ve flushed the permalinks; that’s not the problem. Immediately after the 404, a second attempt usually delivers the file, but our visitors don’t know to try to click twice (and they shouldn’t have to!).
I hear from a contact at the hosting company that lots of their clients are having this problem with download monitor. Is it possible that something can be done in the plugin itself to address this?
