Form\Table?

  • Hello all,
    I am looking at setting up a simple form that has one input and a list box and if need be have it so i can delete anyone or all names that are populated into this list box.

    An example of what i am looking for can be found on my site andyharding.ca/tennis/ (password is wordpress)

    what would be the easiest plugin for me to use that would achieve this?
