Forms will not submit, error every time

Also, I noticed a bug. Adding tooltip works fine, but adding a description for the field to show below causes the entire field to disappear.

Thanks another favor, could you export your form and send it to me please? I think i know what the problem is but i need to check the form configuration to be sure. To send me the file you can use a service like https://wetransfer.com/ (it is free) to upload the file and then send me the link to it.

Regarding the tooltip issue, to keep things organized, could you create a different thread for that please?
