FORMS NOT WORKING !!

My forms were working BUT now when I input all the required info I get a message saying “Your entered code is incorrect.” and then below in a red bordered box it says
“Validation errors occurred. Please confirm the fields and submit it again”
The form is not working and I am not sure why…
Please help!
