Forms not showing in Chrome

My forms – see for example https://ventizo.com/contact/ – are not showing in Google Chrome and Opera:

With Cache Enabler disabled (or the contact page excluded from caching), the forms do show up in Chrome and Opera.

With the following setting unchecked, the forms DO show in all browers.
With the following setting checked, the forms DO NOT show in Chrome and Opera.
