    It looks like your caching plugin (AutoOptimizer) is breaking the JavaScript that outputs the form to the page. Can you try with it disabled and let us know if the problem persists? We’ll look into what might be happening to see if it can be prevented on our end.

    The plugin Cache Enabler – WordPress Cache by KeyCDN holds the following option:

    Create an additional cached version for WebP image support.

    With the setting unchecked, the forms DO show up in all browsers.

    With the setting checked, the forms DO NOT show up in Chrome and Opera.

    Note that, Chrome and Opera support WebP images (the other browsers will follow), and thus are served the cached version for WebP image support.

    Fixed with new update of “Cache Enabler – WordPress Cache” plugin by KeyCDN. I highly recommend this brilliant plugin.
