Forms not displaying on page(s)

Dear reader,

Up until recently the form on the page attached (and other forms on other pages) were displaying just fine. For some reason they no longer are displayed.

I’ve checked this forum for possible solutions and added the CSS code

.forminator-custom-form {
  display: block !important;
}

to Appearance > miscellaneous > custom code in my theme.

Unfortunately this didn’t help. Nor did clearing the cache. Besides the settings for my forminator forms are such that they are excluded from caching.

When I check the page source it displays this message:

DevTools failed to load SourceMap: Could not load content for https://daregreatlycoaching.com/wp-content/plugins/forminator/assets/js/library/jquery.number.min.js.map: HTTP error: status code 404, net::ERR_HTTP_RESPONSE_CODE_FAILURE

How do I get the forms to display correctly again?
