Forms no longer working

I upgraded Constant Contact Plugin to the latest version (4.0.2) and none of the form designer options works. For example, when I click Form Fields dropdowns, nothing drops down. I deactivated and deleted the plugin, then reinstalled and reauthorized it. Still, nothing works.
