    A few days ago, a number of the forms we set up with Forminator on our website stopped working. They do not show up to users on the site and when I look through the backend on the Divi Visual builder, The section where the code is is just flashing like a glitch. I tried to create a duplicate of my form, but when I copied that shortcode the same thing happened.

    When I attempt to create a new form, an error code popped up that said undefined.

    I talked to the Divi support team and gave them remote access and they determined it was not a Divi problem. I also was doing some troubleshooting trying to create a new page in the default editor with both code and simple text and both times got a response saying that the JSON response failed.

    I also had a save fail when i was doing tests within the divi visual builder, so I’m not sure if that is related or if that gives any insight on how to fix the problem.

    If anyone has any suggestions on how to fix the issue it is greatly appreciated.

    I have had problems with forminator in the past namely a few months ago, had some significant problems with email notifications and forms not delivering to the emails we had set in the settings, if that adds any context.

    Hope this message finds you well.

    After visiting the link you shared I can confirm the form is not being created/added, this is usually related to Forminator JS assets not being loaded due to a conflict with other plugins (maybe optimize/caching) or theme.

    Could you let us know which versions of Forminator are you using?

    Also, could you perform a conflict test? Kindly follow this guide https://wpmudev.com/docs/getting-started/getting-support/#conflict-test.

    We strongly suggest you run this test on a staging/dev environment or run a backup before.
