    I hope you are doing good today.

    Are those exact same sites on those URLs? Based on the site source form on testing site is closed in additional div. Are you able to move staging to testing and see does issue will exist as well?

    Also, please deactivate all plugins except Forminator and check if the problem is gone. If so, then enable all plugins one by one and find which one is having a conflict. If there is no positive result, switch to the default WordPress theme like 2019 and see if it works.

    I have tried all these things. I have downloaded the testing site code and set up in my local and I am getting the same issue. Only difference in testing site code is that if we add that short code its creating only this much.

    But in my staging site its creating the actual form.

    I have exported the same form and created a new form that also not displaying.

    I did not see any closed div which you have mentioned. I can see only the above script.

    I am moving the staging changes to testing and the testing site is not working as expected. Could you please help me on this. I am printing some text just above this shortcode that is printing. Could you please send me the closed div which you can see in your end?

    Thank you for the information.

    I checked both pages but I can see the forms, were you able to fix it?

    The Forminator requires jQuery to load the front end scripts, when printing on a custom page we need to use the wp_head and wp_footer allowing WordPress to print all necessary CSS and Scripts.
