Form works on Firefox but not Chrome, Safari or IE

I posted this yesterday, but the problem seemed to resolve itself and now has reverted back to not working on the below browsers.
My form is working well in Firefox but I am having issues with the form not fully submitting in Chrome, Safari and IE.
The Arrow circle spins on and never gives the sent message on browsers other than Firefox.
No changes have been made since it was working and now. Could this be a server issue?
Any help is greatly appreciated!
