Form works intermittently

I have a form defined that has had a few people complete and yet I have only received two copies of the form via email. In messages I can only see two messages but there are a few entries that say Not Paid. However, I have received 5 payments in total through this paypal button.
Also, I am not sure where the files that they upload go to.
