Form won't submit properly in Safari/Chrome

Having some trouble getting a form to submit properly with Safari or Chrome. No e-mail is sent and no data is saved to the database (using Contact form to DB). Everything works fine in IE/Firefox.

https://www.immpressmagazine.com/apply It also doesn’t return any validation errors even if I submit it with nothing filled out.
