Form with Post Action – Post

I’m wondering if anyone has any info about building a form that has a “Form Action” of Post. I would like to build a form for my client that will post the information to a “Mals Cart” which is on online cart processing service hooked into Authorize.net. They need to take deposits for their products. Here is an example:
