Form with CSV and file attachment option

Hello

I am looking for a Form that has the ability to:
+ upload an attachment
+ have filterable drop down menus (similar to a Make/Model/Year)
– user selects Make, then depending on what Make was selected, the Model options will appear
– user now selects Model, then depending on what Model was selected, the year options will appear
– user can now select Year

– the Make/Model/Year options are all fed off of what is put in a CSV file…or equivalent

Gravity Forms has what I am looking for, but I don’t want to pay $199 for it.
