Form will not submit if check boxes are missed

    What other plugins and theme do you use on the site?

    Use a plugin called
    WP Plugin Manager and disable plugins that are not needed on this page in order to judge the problem area.
    If you still have the error then it might have something to do with the

    Uncaught ReferenceError: cntrUpTag is not defined
    <anonymous> https://www.mealsonwheelsmd.org/client-application/:118
    you are getting.

    Now from a UI/UX point of view, for binary decisions, do not use 2 checkboxes here and set them as required. Either use a radio button with a default value set or use a single checkbox which by logic returns no if left unchecked.
