Form Title Defaulting to ‘Mailchimp Signup Form’

I’ve been using the plugin for several months with the Avada theme. I’ve always left the widget ‘Title’ field blank so it does not display on the page. I’ve also selected the Avada option to ‘NOT show the widget title’.
I just noticed the title was showing on the form again, so I removed the field content and saved. But then the content re-populated automatically: ‘Mailchimp Signup Form’ the text of which is then displayed on screen on the live site. Can anyone advise how to remove this and keep it that way please? Thanks
