Form Stopped Working, It Doesn't Redirect Anymore

  • I have been using this form for over a year and now it doesn’t work.

    The form gets filled out, but the lead information is not received.

    I need a way to recover the 10 leads that weren’t received since Nov 2nd.

    Also, I need a way to fix the form.
