Form stopped working as of 1.26

I had a form that was working as of 1.26 and stopped working in 1.27 and 1.28. It is a bit more complicated, as it reads field IDs of the sender and reads an email label from a dropdown of registered users.

Overall, it seems if I try to send notifications to even one email address it breaks. I started recreating the form with most of the functions taken out, it worked, but then once I added in more complex items it broke. If I delete these items and take it back to a basic state, it still won’t work. Any ideas?
