Form starts to render then disappears

WordPress Website https://megan.wolfenden.net has plugin, API code and script added to homepage. Widget added to right side of page. BOTH Forms begin to render then disappears from page. Space is available but nothing visible. Popup blockers and add blockers off. Tried in Safari and Firefox. Not sure what is going on.
