    1. Ive created a Forminator quiz (personality quix). On the ‘Leads’ section i’ve included a Leads form (screenshot)
    2. When i test this on the website the leads form is showing correctly on the last question and before showing the results (screenshot)
    3. However when i check the ‘Submissions’ section then i’m not seeing the lead information. I’m worried that this is not getting captured correctly (screenshot)
    4. I checked the Lead form and noticed the status is set to draft (even though it’s showing on the website correctly). If i publish it from the back end and refresh, the status changes back to draft (no error message is shown). This looks like a bug to me. (Screenshot)

    I’m not able to understand why the lead information (i.e. name and email address) is not getting displayed under the ‘Submissions’ page

    This issue was reported as a bug recently to our developers. Right now there is no hotfix available, although our developers working on it.
    Once the hotfix will be out, we will post it in this thread.

    Is the issue just that the details are not getting displayed? Or is it that it’s not getting collected at all?

    For example, if I setup integration with another tool, will the lead information be sent to the tool?

    Thanks for response!

    Getting a step back: the form “stuck” in “draft” mode is correct. This is a “special” form and it’s automatically set to “draft” so it can’t be used separately “on its own” as it integrates with the quiz.

    As for your question: you can set any of available integrations for the quiz and the lead data would be submitted to it, regardless of the bug.

    I just tested it on my own test site with MailChimp as example and fields gathered by the lead form of my quiz are correctly submitted to my MailChimp audience.

    Note though: the integration must be set with the quiz, not with the lead form itself though the form must of course still be there.

    @wpmudev-support8 thank you for this clarification.

    – this understanding came from the YouTube video here where it shows the lead form in ‘published’ state.

    I will try using Zapier integration with the quiz to see if the lead form information is received. FYI we are using ConvertKit as our marketing tool

    Do you have an ETA on when you expect the bug to be resolved (i.e. the lead information to be displayed under the submissions page)?

    I’m afraid, at the moment there isn’t any exact ETA that we could provide regarding the status of a fix.

    I can confirm our team has confirmed the issue and it’s already being looked at. Hopefully, it’ll be soon.
