Form questions in slides: How do I replicate this functionality?

  • Hi guys,
    Anyone know how I could build a form into multiple slides like so:
    https://howmuchdoesawebsiteco.st/

    It does not need to do any calculations. It is just a contact form with each question on its own slide.

    So
    slide 1:
    first question

    slide 2:
    another question

    Slide 3:
    Last question + Send.
