Form Processor Boilerplate not working as expected with latest version

I’ve been working on a Form Processor. I’ve tried to run the form processor using the boilerplate and the type error is coming up, e.g. where you setup the type=”error” and “note” => “Nope Sorry”. That part works, but the “fields” part isn’t working anymore. It was working on an earlier version when I was coding it (before updating to the latest version of the plugin). It’s supposed to add a field error to the “second_option” when you enter the word “die”.
