Form plugin to put data into MySQL

  • I’m looking for a plugin that I can include on any page, where people can select options from several drop-down menus, which then send this data entered by users to my database. Later I’d like to retrieve that data to create a graph that will be displayed on the site.

    But for now I’m looking for a form that works. Does anyone know a good one? I couldn’t really find what I’m looking for. Some plugins haven’t been updated in ages also…
