Form Plugin

Seems to me that almost any form plugin could do this. My favorite is Formidable, but check the link.

the paid one or the free one. is there a way to edit form code to do that

Ask in their support forum here. If by “another form page”, also be sure to check the plugin descriptions for multi-part forms.
