Form Not Working on Mac

  • Pwd: 12title34
    Form works well on windows chrome but not on mac chrome. File upload will not work. On safari there are problems with the required fields (errors saying field is empty when it is not). If I can just get this fixed on mac chrome I can live with it. Any suggestions?
