Form not working after inserting code into my custom code on page.php

    Hi,

    I have a custom code that I inserted into page.php for a client. I added the form php code and the form shows up but it won’t send any email. It used to work with the template I had installed before on wordpress. Since I put in a custom code it won’t work. I had installed some smtp email thing. ( I don’t know much about coding). The client just wants to collect emails (a one field form). How can I fix this? Also is there a way that the text can be removed once you click in the box?

    Dear user,

    Please make sure you have used the php code option when inserting the Form into the page. It can be found in Manager section of the Form Maker to the right of the form. If you have inserted that code and still face the issue please contact us at info[at]web-dorado.com. As for the pre-filled text to be removed, you should put that text in Value if Empty option, instead of writing it straight into the form. Thank you.
