Form Not Working

  • No ne including me can submit the form on any device keep getting this message:

    Validation errors occurred. Please confirm the fields and submit it again.

    form is filled out correctly please help!

    I only added this form about 3 weeks ago! i tried and update link I found in the forum but it had an error message.

    the site is starcitymusicnotes.com but I deleted the plugin! I have to find something ASAP that works consistantly because of the nature of the site. it was a really nice plugin while it worked!
