Form not showing up

I was able to replicate the issue on the page that you shared. Upon further checks I noticed the issue is happening due to a CSS rule being applied to the form.

However, since you’re able to see the form while editing the page and even when you preview it, this could mean that the issue is most probably not related to custom CSS however I am not sure about it as yet because it needs more checks.

Could you please verify if you’ve set the form to be visible only for “logged-in users”?

You can check this by navigating to WP admin >> Forminator >> Edit form >> Behaviour >> Security >> Review Enable Logged in submission only.

If yes, please disable the toggle save the form and check if that helps.

Could you please check and see what option you have enabled under “Rendering” as shown in the following screenshot:

If “Load form using AJAX” & “Prevent page caching on form pages” is disabled, could you please check and see how it behaves once enabled and also vice versa if its already enabled?

If the issue is still the same even after that then possible to also share the form export, so that we could have a better idea?

If you are concerned about any sensitive information in the form, then you can duplicate your form, remove any sensitive information, and then export it.

You can share the export file via Google Drive, Dropbox or any cloud service in the next reply.
