Form not showing in any browsers

  • Operating System: Windows 8.1
    Browsers
    Chrome – Version 49.0.2623.110 m
    IE – Version 11.0.9600.18231
    Firefox – Version 45.0.1

    Using [jotform id=”<form_id>”] results in a blank content area, where the form should be.

    On initial load, IE behaves same as the other browsers; when I refresh the page, however, the form shows up for a split second then goes away.
