I’ve imported the form to my test site and tried to submit it. I’ve only filled-in required fields but the form submitted just fine and I’ve also received notification to my inbox – to the e-mail address that I’ve set in notification.
Then I’ve also tried to submit the form directly on your site and this also submitted fine. I didn’t receive any e-mail about it but that’s because form only has a single notification e-mail configured – set to send e-mail to fixed e-mail address (that’s “admin notification”), specifically to “enq…@..og.uk” address”.
That being said, by “not sending” do you mean that the form itself doesn’t get submitted when you try to do it – if so, is it displaying any errors upon submission (and what are they)?
Or is it that you are not receiving e-mails? If so – have you checked spam inbox to make sure they are not there? Are other e-mails form your site (e.g. notifications from other plugins or lost password e-mails) delivered fine?

Also, if it’s about the user not getting e-mail (the user who submits the form) then it’s a different thing and it only requires creating additional notification in form’s “Email notifications” settings, where for “recipient” you would use an email field (like {email-1}) of the form instead of fixed e-mail address.
