Form not rendering

Form no longer renders, and when I try to go to the plugin settings page in WP-admin, i get a 502 error. We’ve filed a ticket with Salesforce to ensure IP Whitelisting, but there’s nothing in console, and we had fixes XSS issues that presented similarly in the past.
