Form not loading

Has anyone had the issue where the page the form is on will not load? In the documentation it says the theme needs to run at least jquery 1.7 and my theme Salient is running 1.11. I am guessing it is a conflict with the theme but not sure where to start to troubleshoot and correct the issue. Any suggestions would be greatly appreciated.
