Form not getting submitted due to upload field

You can configure a temporary page for the upload form and send it.

Apologies for the inconvenience, not sure why the short URL did not work.

This form is a test form and has only 2 fields, name and upload.

Sorry for the delay on this.

The form submit is returning a fatal error:

This kind of error generates a log line on WordPress debug.log file.

Could you please enable the debug mode on the WordPress following this guide:
https://premium.wpmudev.org/blog/debugging-wordpress-how-to-use-wp_debug/

Once done, submit the form again, it will return the same problem but this time check the wp-content folder for a debug.log file.

Download it, upload on the Google drive and share the link.
