Form not displaying

Similar to other people who have posted, the problem began with the form not showing up on the front end (but still displayed on the back end). Deactivated other plugins and the problem still persists.
However, now the form does not display on the back end of the website – either in the page editor where the form is supposed to be or the form editor.
In the form editor, it says there are 7 fields in the form. But when I click on Edit, nothing populates in the Preview field.
Please help!
