Form not diplaying on page

  • When I use this shortcode, the form shows fine:

    [formassembly formid=”4913666″ server=”https://tfaforms.com” iframe=1]

    However, when I use any of these, an empty paragraph is displayed instead of the form:

    [formassembly formid=”4913666″ server=”https://tfaforms.com”%5D
    [formassembly formid=”4913666″]
    [formassembly formid=4913666]

    Can you help me fix this issue so that I can publish the form without an iframe?
