form maker is not working with page template

    Hi,
    I am trying to embedded short code to custom page template , I tried from editor to add form code – [Form id=”1″] as well tried from code <?php echo do_shortcode( ‘[Form id=”1″]’ ); ?> but in both case getting issue such as :
    1. getting un label field on the top of the form
    2. submit and reset button are now working and distorted.
    3. form is not working on submit and reset.

    while this form is working on default post template but not on page template

    Thanks for your inquiry.

    Dear friend, this seems to be a conflict with the theme or other plugins, the js and CSS files are not included.

    Please try to troubleshoot the conflict with HealthCheck plugin.
