Form Maker Form Not Being Submitted

Hi All,

This just started today – everything else has been fine up until late this morning. Right now, when someone hits the “Done” button after filling out the form, nothing is submitted. The form reloads with all the data that’s been filled in remaining on the screen. When it worked, confirmation emails were sent to the subscriber and to me and it would redirect to a our homepage.

Nothing was changed on the backend, so I’m a little perplexed as to why this is happening. Any suggestions or ideas would be appreciated!
