Form Maker – Conditional logic not working and options not changing

I tried the free version of Form Maker and it worked great. Upgraded to the Pro version and it worked great. Then it went a little wonky – I think it was after I installed W3 total Cache to speed up my website because my speed scores weren’t good.
So testing that hypothesis I deactivated W3 and tested Form Maker again. Form Maker still was having problems – so I deleted W3 and the Form Maker problems were still there.
I also added an SSL certificate to the site after I installed Form Maker Pro – but I wouldn’t think that would be a problem.
So then I deactivated and uninstalled Form Maker, re-installed it and activated it again. The problems are still there.
Here’s problem #1 – Options – I insert a new field and type in a name on the option but the value doesn’t change. But I do have a workaround for that – I just delete the supplied options and add new options with the +option buttons and it works.
Problem #2 – Conditional Logic – I have a multiple choice field with 10 different options to check (near the bottom of the form on the above page – How would you describe the shamans you’ve worked with and their training). Of the 10 fields I have 7 pop-ups I need to add. The fields that are supposed to pop up will work if I add all 7 options and pop-up fields at once and then update. But here’s the strange part – if I do that sometimes other pop-up fields unrelated to the 10 option field will quit working after I update for the entire 10 option question.
If I add just one or two pop-up fields to the 10 option field and update those two will work. If I then add a third pop-up field to the 7 option field all of them will quit working or just the last one I added will work – sometimes none of them will work. It varies.
So in the sample page on the 10 option question at this point the 5th option pop up works, but the previous pop-ups for the fields above that one quit working.
I am at the end of my trouble-shooting abilities. Any help would be GREATLY appreciated. I can get around in Word Press and install plugins and use them but that’s about it. So thanks in advance,
