Form is not displaying.

Thanks for your inquiry.

Please add the below code in Appearance > customize > additional CSS section and the issue will be solved:

.fm-form-container{
display:block !important
}

Please note that now only one form we see added on that page, have you removed the other 2?

Please keep me updated with the results.
