Form is messing up alignment of other items on pages.

I have recently installed this FormGet plugin onto my website. However once I activated it and it appeared on my website, everything else on the pages have shifted so now the alignment and position of my content is off. All my content was centered on each page, but now it appears the form has pushed everything to the right a bit, which is making some items move down the next line.
Basically my question is, is there a way to have this form but not have it affect everything else on my page? The link to our website is https://www.tsihealthcare.com/maintesting . You will notice the content on the pages is off andthe top header with the logo and secondary menu is all off now. Its like there was a random margin added, but I’m not sure.
If you could get back with me asap, I would greatly appreciate it. This alignment issue makes it really hard to work on the website because I won’t know what my changes actually look like. Thanks.
