Form information not showing on email

    Hi, I′ve set up you plugin according to the recommendations and tutorials. I′m having a problem with the form, I′m not recieving the information that the users write on each field, the email message arrives like this:

    Nombre y Apellido del padre (obligatorio)pual —>>OK

    Correo electrónico (obligatorio)[email protected] —>>OK

    Celular que vas a usar para hacer tus consultas (obligatorio)2345667 —>>OK

    Selecciona cuantos hijos tienes para crear sus perfiles (obligatorio)1 —>>OK CONDITIONAL THE OPTIONS GO FROM 1 TO 5.

    Nombre de tu hija/o #1 (opcional) —NOTHING
    Fecha de nacimiento hija/o #1 (obligatorio) —NOTHING
    Peso (en Kg) hija/o #1 (obligatorio) —NOTHING
    Vacunas al día hija/o #1 (obligatorio)
    Si —NOTHING, DEFAULT VALUE
    Enfermedades de importancia hija/o #1 (obligatorio) —NOTHING
    Alergias hija/o #1 (obligatorio) —NOTHING
    Medicamentos hija/o #1 (obligatorio) —NOTHING
    Algo adicional que nos quieras contar de tu hija/o #1 (opcional) —NOTHING

    When we insert information for the option that has 5 kids the message arrieves with all the information.

    sorry to disappoint you. very busy with paid work at the moment. seems like you are using duplicate field names. can you check? please also check the sticky post to solve common problems
