form header showing on site

  • I installed a template from Template Monster for a client… I could not get the Contact 7 form to function so I thought I’d add my own….but now the form header is showing on the site.

    So if someone can either

    • show me how to get Contact 7 to function using checkboxes
    • OR show me how to get my custom form to show correctly on the site

    Can you tell me … How far were you able to get with Contact Form 7?

    I can get the fields to work…but can not get the checkboxes to line up one above the other, or one after the other. Also, when sending the form NONE of the info is going to the email address listed.

    I’d like to be able to change the size of the area the description for each field is in but that’s 2nd to the above needs.
