Form from Your Plugin shows in Gravity Forms Editing

Hi,
Your plugin is installed and active. When editing Gravity Forms, and scrolling all the way to the bottom of the screen, I see maybe 20% of the right side of a form that almost looks like part of a embed any doc form.
Here are excerpts I was able to copy:
