Form Formatting Only Working on Chrome

    My form only looks as designed on Chrome. Both Microsoft Edge and Firefox appear a jumbled mess. Any idea how to fix this?

    The mentioned issue with the form persists with all browsers. Kindly try to disable the cache plugin in order to see whether the mentioned issue persists. The latter may be the result of the minifying.
