Form for an Hotel website

I am working in a website for a hotel. I need a form with followings fields:
– From (date) (for example 01/01/2016)
– To (date) (for example 10/01/2016)
– And a button “Booking”When button “Booking” will be pressed, a new page will be open. For example,
https://www.mybookings.com/bookings.jsp?DateIn=01/01/2016&DateOut=10/01/2016How can I do this? Any idea?
